This position is responsible for supervising the personal care staff, coordinating all personal care services provided to residents by personal care staff, promoting the health and wellbeing of each resident, and reporting observed changes in resident condition or health to the Executive Director and resident’s responsible party

Responsibilities

  • Perform thorough Health Care Practitioner Physical assessment on new residents.
  • Monitor the health status of all current residents.
  • Create and ongoing verification of current medication profile.
  • Perform review of resident assessments and care plans every 45 days for staff compliance.
  • Assesse and make determination of appropriate nursing staff team members upon hire and every 45 days to ensure appropriate care to resident of all levels.
  • Delegate responsibility and supervision of nursing tasks based on determination.
  • Ensure accurate documentation of resident condition and care.
  • Responsible for teaching Medication Technician Training program.
  • Foster teamwork with co-workers, show initiative in overcoming obstacles, take actions which assist others beyond job responsibilities, and have a positive impact on co-workers, residents, and visitors.
  • Perform other duties as assigned or needed.

Qualifications

  • Bachelor’s degree in nursing strongly preferred.
  • Current Registered Nurse License required. State specific requirements, licenses and certifications as applicable.
  • Must have proven management experience and ability to manage staff.
  • Minimum three years’ experience in Assisted Living, Alzheimer’s, or Memory Loss care preferred.
  • Experience as Residential Care Management, Clinical Director, or Risk Management preferred.
  • Experience with Microsoft Word and Excel preferred.
